Move the stable_writes flag into the queue_limits feature field so that
it can be set atomically with the queue frozen.
The flag is now inherited by blk_stack_limits, which greatly simplifies
the code in dm, and fixed md which previously did not pass on the flag
set on lower devices.

Nothing prevents iscsi_sw_tcp_conn_bind() to receive file descriptor
pointing to non TCP socket (af_unix for example).
Return -EINVAL if this is attempted, instead of crashing the kernel.

The validity of sock should be checked before assignment to avoid incorrect
values. Commit 57569c37f0ad ("scsi: iscsi: iscsi_tcp: Fix null-ptr-deref
while calling getpeername()") introduced this change which may lead to
inconsistent values of tcp_sw_conn->sendpage and conn->datadgst_en.
Fix the issue by moving the position of the assignment.

If during iscsi_sw_tcp_session_create() iscsi_tcp_r2tpool_alloc() fails,
userspace could be accessing the host's ipaddress attr. If we then free the
session via iscsi_session_teardown() while userspace is still accessing the
session we will hit a use after free bug.
Set the tcp_sw_host->session after we have completed session creation and
can no longer fail.

Fix the above bug by splitting the session removal into 2 parts:
1. removal from iSCSI class which includes sysfs and removal from host
tracking.
2. freeing of session.
During iscsi_tcp host and session removal we can remove the session from
sysfs then remove the host from sysfs. At this point we know userspace is
not accessing the kernel via sysfs so we can free the session and host.

Since moving to memalloc_nofs_save/restore, SUNRPC has stopped setting the
GFP_NOIO flag on sk_allocation which the networking system uses to decide
when it is safe to use current->task_frag. The results of this are
unexpected corruption in task_frag when SUNRPC is involved in memory
reclaim.
The corruption can be seen in crashes, but the root cause is often
difficult to ascertain as a crashing machine's stack trace will have no
evidence of being near NFS or SUNRPC code. I believe this problem to
be much more pervasive than reports to the community may indicate.
Fix this by having kernel users of sockets that may corrupt task_frag due
to reclaim set sk_use_task_frag = false. Preemptively correcting this
situation for users that still set sk_allocation allows them to convert to
memalloc_nofs_save/restore without the same unexpected corruptions that are
sure to follow, unlikely to show up in testing, and difficult to bisect.

Fix a NULL pointer crash that occurs when we are freeing the socket at the
same time we access it via sysfs.
The problem is that:
1. iscsi_sw_tcp_conn_get_param() and iscsi_sw_tcp_host_get_param() take
the frwd_lock and do sock_hold() then drop the frwd_lock. sock_hold()
does a get on the "struct sock".
2. iscsi_sw_tcp_release_conn() does sockfd_put() which does the last put
on the "struct socket" and that does __sock_release() which sets the
sock->ops to NULL.
3. iscsi_sw_tcp_conn_get_param() and iscsi_sw_tcp_host_get_param() then
call kernel_getpeername() which accesses the NULL sock->ops.
Above we do a get on the "struct sock", but we needed a get on the "struct
socket". Originally, we just held the frwd_lock the entire time but in
commit bcf3a2953d36 ("scsi: iscsi: iscsi_tcp: Avoid holding spinlock while
calling getpeername()") we switched to refcount based because the network
layer changed and started taking a mutex in that path, so we could no
longer hold the frwd_lock.
Instead of trying to maintain multiple refcounts, this just has us use a
mutex for accessing the socket in the interface code paths.

For software iSCSI, we do a session per host so there is no need to set the
target's can_queue since it's the same as the host one. Setting it just
results in extra atomic checks in the main I/O path.

We don't always want to run the recv path from the network softirq because
when we have to have multiple sessions sharing the same CPUs, some sessions
can eat up the NAPI softirq budget and affect other sessions or users.
Allow us to queue the recv handling to the iscsi workqueue so we can have
the scheduler/wq code try to balance the work and CPU use across all
sessions' worker threads.
Note: It wasn't the original intent of the change but a nice side effect is
that for some workloads/configs we get a nice performance boost. For a
simple read heavy test:
fio --direct=1 --filename=/dev/dm-0 --rw=randread --bs=256K
--ioengine=libaio --iodepth=128 --numjobs=4
where the iscsi threads, fio jobs, and rps_cpus share CPUs we see a 32%
throughput boost. We also see increases for small I/O IOPs tests but it's
not as high.

When the system is shutting down, iscsid is not running so we will not get
a response to the ISCSI_ERR_INVALID_HOST error event. The system shutdown
will then hang waiting on userspace to remove the session.
This has libiscsi force the destruction of the session from the kernel when
iscsi_host_remove() is called from a driver's shutdown callout.
This fixes a regression added in qedi boot with commit d1f2ce77638d ("scsi:
qedi: Fix host removal with running sessions") which made qedi use the
common session removal function that waits on userspace instead of rolling
its own kernel based removal.

Instead of storing the iSCSI task pointer and the session age in the SCSI
pointer, use command-private variables. This patch prepares for removal of
the SCSI pointer from struct scsi_cmnd.

Userspace (open-iscsi based tools at least) sets no linger on the socket to
prevent stale data from being sent. However, with the in-kernel cleanup if
userspace is not up the sockfd_put will release the socket without having
set that sockopt.
iscsid sets that opt at socket close time, but it seems ok to set this at
setup time in the kernel for all tools.

We are setting the shost's can_queue after we add the host which is too
late, because the SCSI midlayer will have allocated the tag set based on
the can_queue value at that time. This patch has us use the
iscsi_host_get_max_scsi_cmds() helper to figure out the number of SCSI
cmds.
It also fixes up the template can_queue so it reflects the max SCSI cmds we
can support like how other drivers work.

The kernel may fail to boot or devices may fail to come up when
initializing iscsi_tcp devices starting with Linux 5.8.
Commit a79af8a64d39 ("[SCSI] iscsi_tcp: use iscsi_conn_get_addr_param
libiscsi function") introduced getpeername() within the session spinlock.
Commit 1b66d253610c ("bpf: Add get{peer, sock}name attach types for
sock_addr") introduced BPF_CGROUP_RUN_SA_PROG_LOCK() within getpeername(),
which acquires a mutex and when used from iscsi_tcp devices can now lead to
"BUG: scheduling while atomic:" and subsequent damage.
Ensure that the spinlock is released before calling getpeername() or
getsockname(). sock_hold() and sock_put() are used to ensure that the
socket reference is preserved until after the getpeername() or
getsockname() complete.

The BDI_CAP_STABLE_WRITES is one of the few bits of information in the
backing_dev_info shared between the block drivers and the writeback code.
To help untangling the dependency replace it with a queue flag and a
superblock flag derived from it. This also helps with the case of e.g.
a file system requiring stable writes due to its own checksumming, but
not forcing it on other users of the block device like the swap code.
One downside is that we an't support the stable_pages_required bdi
attribute in sysfs anymore. It is replaced with a queue attribute which
also is writable for easier testing.

A faulty userspace that calls destroy_session() before destroying the
connections can trigger the failure. This patch prevents the issue by
refusing to destroy the session if there are outstanding connections.

If a faulty initiator fails to bind the socket to the iSCSI connection
before emitting a command, for instance, a subsequent send_pdu, it will
crash the kernel due to a null pointer dereference in sock_sendmsg(), as
shown in the log below. This patch makes sure the bind succeeded before
trying to use the socket.

The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers. Note that in many cases the setting might
be bogus, but this keeps the status quo.

iscsi tcp will first send out data, then calculate and send data
digest. If we don't have BDI_CAP_STABLE_WRITES, the page cache will be
written in spite of the on going writeback. Consequently, wrong digest
will be got and sent to target.
To fix this, set BDI_CAP_STABLE_WRITES when data digest is enabled
in iscsi_tcp .slave_configure callback.

All these functions either return a negative error indicator,
or store length of sockaddr into "int *socklen" parameter
and return zero on success.
"int *socklen" parameter is awkward. For example, if caller does not
care, it still needs to provide on-stack storage for the value
it does not need.
None of the many FOO_getname() functions of various protocols
ever used old value of *socklen. They always just overwrite it.
This change drops this parameter, and makes all these functions, on success,
return length of sockaddr. It's always >= 0 and can be differentiated
from an error.
Tests in callers are changed from "if (err)" to "if (err < 0)", where needed.
rpc_sockname() lost "int buflen" parameter, since its only use was
to be passed to kernel_getsockname() as &buflen and subsequently
not used in any way.
Userspace API is not changed.

We now have memalloc_noreclaim_{save,restore} helpers for robust setting
and clearing of PF_MEMALLOC. Let's convert the code which was using the
generic tsk_restore_flags(). No functional change.

current_restore_flags()
It is not safe for one thread to modify the ->flags
of another thread as there is no locking that can protect
the update.
So tsk_restore_flags(), which takes a task pointer and modifies
the flags, is an invitation to do the wrong thing.
All current users pass "current" as the task, so no developers have
accepted that invitation. It would be best to ensure it remains
that way.
So rename tsk_restore_flags() to current_restore_flags() and don't
pass in a task_struct pointer. Always operate on current->flags.

Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates. This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

iscsi_sw_tcp_data_ready() and iscsi_sw_tcp_state_change() were
using read_lock(&sk->sk_callback_lock) which is fine if caller
disabled BH.
TCP stack no longer has this requirement and can run from
process context.
Use read_lock_bh() variant to restore previous assumption.
Ideally this code could use RCU instead...

Drop the now unused reason argument from the ->change_queue_depth method.
Also add a return value to scsi_adjust_queue_depth, and rename it to
scsi_change_queue_depth now that it can be used as the default
->change_queue_depth implementation.

All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.
Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

Define separate fields in the sock structure for configuring disabling
checksums in both TX and RX-- sk_no_check_tx and sk_no_check_rx.
The SO_NO_CHECK socket option only affects sk_no_check_tx. Also,
removed UDP_CSUM_* defines since they are no longer necessary.
