Commit e3e9bda38e6d ("s390/virtio_ccw: use DMA handle from DMA API")
broke configuration change notifications for virtio-ccw by putting the
DMA address of *indicatorp directly into ccw->cda disregarding the fact
that if !!(vcdev->is_thinint) then the function
virtio_ccw_register_adapter_ind() will overwrite that ccw->cda value
with the address of the virtio_thinint_area so it can actually set up
the adapter interrupts via CCW_CMD_SET_IND_ADAPTER. Thus we end up
pointing to the wrong object for both CCW_CMD_SET_IND if setting up the
adapter interrupts fails, and for CCW_CMD_SET_CONF_IND regardless
whether it succeeds or fails.
To fix this, let us save away the dma address of *indicatorp in a local
variable, and copy it to ccw->cda after the "vcdev->is_thinint" branch.

Instead of converting virtual to physical addresses with the virt_to_dma*()
functions, use dma addresses as provided by DMA API and only add offsets to
these addresses. This makes sure that address conversion is only done by
the DMA API.

Change and use ccw_device_dma_zalloc() so it returns a virtual address like
before, which can be used to access data. However also pass a new dma32_t
pointer type handle, which correlates to the returned virtual address.
This pointer is used to directly pass/set the DMA handle as returned by the
DMA API.

Remove the field `lsi_mask` from `struct airq_struct` as it is not
utilized for any adapter interrupt, other than setting it to the default
value of 0xff.
Because nobody is using this functionality, all it does is cost a little
bit of time with each delivered adapter interrupt.

According to VirtIO spec v1.2, VIRTIO_F_NOTIFICATION_DATA feature
indicates that the driver passes extra data along with the queue
notifications.
In a split queue case, the extra data is 16-bit available index. In a
packed queue case, the extra data is 1-bit wrap counter and 15-bit
available index.
Add support for this feature for MMIO, channel I/O and modern PCI
transports.

This reverts commit a10fba0377145fccefea4dc4dd5915b7ed87e546: the
proposed API isn't supported on all transports but no
effort was made to address this.
It might not be hard to fix if we want to: maybe just
rename size to size_hint and make sure legacy
transports ignore the hint.
But it's not sure what the benefit is in any case, so
let's drop it.

find_vqs() adds a new parameter sizes to specify the size of each vq
vring.
NULL as sizes means that all queues in find_vqs() use the maximum size.
A value in the array is 0, which means that the corresponding queue uses
the maximum size.
In the split scenario, the meaning of size is the largest size, because
it may be limited by memory, the virtio core will try a smaller size.
And the size is power of 2.

virtio-net can display the maximum (supported by hardware) ring size in
ethtool -g eth0.
When the subsequent patch implements vring reset, it can judge whether
the ring size passed by the driver is legal based on this.

When doing device passthrough where interrupts are being forwarded from
host to guest, we wish to use a pinned section of guest memory as the
vector (the same memory used by the guest as the vector). To accomplish
this, add a new parameter for airq_iv_create which allows passing an
existing vector to be used instead of allocating a new one. The caller
is responsible for ensuring the vector is pinned in memory as well as for
unpinning the memory when the vector is no longer needed.
A subsequent patch will use this new parameter for zPCI interpretation.

A subsequent patch will introduce an airq handler that requires additional
TPI information beyond directed vs floating, so pass the entire tpi_info
structure via the handler. Only pci actually uses this information today,
for the other airq handlers this is effectively a no-op.

We try to harden virtio device notifications in 8b4ec69d7e09 ("virtio:
harden vring IRQ"). It works with the assumption that the driver or
core can properly call virtio_device_ready() at the right
place. Unfortunately, this seems to be not true and uncover various
bugs of the existing drivers, mainly the issue of using
virtio_device_ready() incorrectly.
So let's add a Kconfig option and disable it by default. It gives
us time to fix the drivers and then we can consider re-enabling it.

This is a rework on the previous IRQ hardening that is done for
virtio-pci where several drawbacks were found and were reverted:
1) try to use IRQF_NO_AUTOEN which is not friendly to affinity managed IRQ
that is used by some device such as virtio-blk
2) done only for PCI transport
The vq->broken is re-used in this patch for implementing the IRQ
hardening. The vq->broken is set to true during both initialization
and reset. And the vq->broken is set to false in
virtio_device_ready(). Then vring_interrupt() can check and return
when vq->broken is true. And in this case, switch to return IRQ_NONE
to let the interrupt core aware of such invalid interrupt to prevent
IRQ storm.
The reason of using a per queue variable instead of a per device one
is that we may need it for per queue reset hardening in the future.
Note that the hardening is only done for vring interrupt since the
config interrupt hardening is already done in commit 22b7050a024d7
("virtio: defer config changed notifications"). But the method that is
used by config interrupt can't be reused by the vring interrupt
handler because it uses spinlock to do the synchronization which is
expensive.

This patch tries to implement the synchronize_cbs() for ccw. For the
vring_interrupt() that is called via virtio_airq_handler(), the
synchronization is simply done via the airq_info's lock. For the
vring_interrupt() that is called via virtio_ccw_int_handler(), a per
device rwlock is introduced and used in the synchronization method.

CCW_CMD_READ_STATUS was introduced with revision 2 of virtio-ccw,
and drivers should only rely on it being implemented when they
negotiated at least that revision with the device.
However, virtio_ccw_get_status() issued READ_STATUS for any
device operating at least at revision 1. If the device accepts
READ_STATUS regardless of the negotiated revision (which some
implementations like QEMU do, even though the spec currently does
not allow it), everything works as intended. While a device
rejecting the command should also be handled gracefully, we will
not be able to see any changes the device makes to the status,
such as setting NEEDS_RESET or setting the status to zero after
a completed reset.
We negotiated the revision to at most 1, as we never bumped the
maximum revision; let's do that now and properly send READ_STATUS
only if we are operating at least at revision 2.

The access to airq_areas was racy ever since the adapter interrupts got
introduced to virtio-ccw, but since commit 39c7dcb15892 ("virtio/s390:
make airq summary indicators DMA") this became an issue in practice as
well. Namely before that commit the airq_info that got overwritten was
still functional. After that commit however the two infos share a
summary_indicator, which aggravates the situation. Which means
auto-online mechanism occasionally hangs the boot with virtio_blk.

Before virtio-ccw could get away with not using DMA API for the pieces of
memory it does ccw I/O with. With protected virtualization this has to
change, since the hypervisor needs to read and sometimes also write these
pieces of memory.
The hypervisor is supposed to poke the classic notifiers, if these are
used, out of band with regards to ccw I/O. So these need to be allocated
as DMA memory (which is shared memory for protected virtualization
guests).
Let us factor out everything from struct virtio_ccw_device that needs to
be DMA memory in a satellite that is allocated as such.
Note: The control blocks of I/O instructions do not need to be shared.
These are marshalled by the ultravisor.

As virtio-ccw devices are channel devices, we need to use the
dma area within the common I/O layer for any communication with
the hypervisor.
Note that we do not need to use that area for control blocks
directly referenced by instructions, e.g. the orb.
It handles neither QDIO in the common code, nor any device type specific
stuff (like channel programs constructed by the DASD driver).
An interesting side effect is that virtio structures are now going to
get allocated in 31 bit addressable storage.

Currently virtio-ccw devices do not work if the device has
VIRTIO_F_IOMMU_PLATFORM. In future we do want to support DMA API with
virtio-ccw.
Let us do the plumbing, so the feature VIRTIO_F_IOMMU_PLATFORM works
with virtio-ccw.
Let us also switch from legacy avail/used accessors to the DMA aware
ones (even if it isn't strictly necessary), and remove the legacy
accessors (we were the last users).

The commit 2a2d1382fe9d ("virtio: Add improved queue allocation API")
establishes a new way of allocating virtqueues (as a part of the effort
that taught DMA to virtio rings).
In the future we will want virtio-ccw to use the DMA API as well.
Let us switch from the legacy method of allocating virtqueues to
vring_create_virtqueue() as the first step into that direction.

A queue with a capacity of zero is clearly not a valid virtio queue.
Some emulators report zero queue size if queried with an invalid queue
index. Instead of crashing in this case let us just return -ENOENT. To
make that work properly, let us fix the notifier cleanup logic as well.

Some vqs may not need to be allocated when their related feature bits
are disabled. So callers may pass in such vqs with "names = NULL".
Then we skip such vq allocations.

Several conflicts, seemingly all over the place.
I used Stephen Rothwell's sample resolutions for many of these, if not
just to double check my own work, so definitely the credit largely
goes to him.
The NFP conflict consisted of a bug fix (moving operations
past the rhashtable operation) while chaning the initial
argument in the function call in the moved code.
The net/dsa/master.c conflict had to do with a bug fix intermixing of
making dsa_master_set_mtu() static with the fixing of the tagging
attribute location.
cls_flower had a conflict because the dup reject fix from Or
overlapped with the addition of port range classifiction.
__set_phy_supported()'s conflict was relatively easy to resolve
because Andrew fixed it in both trees, so it was just a matter
of taking the net-next copy. Or at least I think it was :-)
Joe Stringer's fix to the handling of netns id 0 in bpf_sk_lookup()
intermixed with changes on how the sdif and caller_net are calculated
in these code paths in net-next.
The remaining BPF conflicts were largely about the addition of the
__bpf_md_ptr stuff in 'net' overlapping with adjustments and additions
to the relevant data structure where the MD pointer macros are used.

While ccw_io_helper() seems like intended to be exclusive in a sense that
it is supposed to facilitate I/O for at most one thread at any given
time, there is actually nothing ensuring that threads won't pile up at
vcdev->wait_q. If they do, all threads get woken up and see the status
that belongs to some other request than their own. This can lead to bugs.

This race normally does not cause any problems. The operations provided
by struct virtio_config_ops are usually invoked in a well defined
sequence, normally don't fail, and are normally used quite infrequent
too.
Yet, if some of the these operations are directly triggered via sysfs
attributes, like in the case described by the referenced bug, userspace
is given an opportunity to force races by increasing the frequency of the
given operations.
Let us fix the problem by ensuring, that for each device, we finish
processing the previous request before starting with a new one.

Currently we have a race on vcdev->config in virtio_ccw_get_config() and
in virtio_ccw_set_config().
This normally does not cause problems, as these are usually infrequent
operations. However, for some devices writing to/reading from the config
space can be triggered through sysfs attributes. For these, userspace can
force the race by increasing the frequency.

Currently, ccw, vop and remoteproc need some legacy virtio
APIs to create or access virtio rings, which are not supported
by packed ring. So disable packed ring on these transports
for now.

Suspend/Resume to/from disk currently fails. Let us wire
up the necessary callbacks. This is mostly just forwarding
the requests to the virtio drivers. The only thing that
has to be done in virtio_ccw itself is to re-set the
virtio revision.

Now that the SPDX tag is in all drivers/s390/ files, that identifies the
license in a specific and legally-defined manner. So the extra GPL text
wording can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/s390/virtio/ files with the correct SPDX license
identifier based on the license text in the file itself. The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

There is no recent user space application available anymore which still
supports this old virtio transport. Additionally, commit 3b2fbb3f06ef
("virtio/s390: deprecate old transport") introduced a deprecation message
in the driver, and apparently nobody complained so far that it is still
required. So let's simply remove it.

The feature member of virtio_feature_desc contains little endian
values, given that it contents will be converted with
le32_to_cpu(). The "wrong" __u32 type leads to the sparse warnings
below.
In order to avoid them, use the correct __le32 type instead.

Add a struct irq_affinity pointer to the find_vqs methods, which if set
is used to tell the PCI layer to create the MSI-X vectors for our I/O
virtqueues with the proper affinity from the start. Compared to after
the fact affinity hints this gives us an instantly working setup and
allows to allocate the irq descritors node-local and avoid interconnect
traffic. Last but not least this will allow blk-mq queues are created
based on the interrupt affinity for storage drivers.
