Rename internal names to reflect the driver's new reference. The module
name remains the same.
Also fix trivial coding style issues on the way related to e.g. alignment
changes due to the rename.

Use the CCS limit definitions instead of the SMIA ones. This allows
accessing CCS capabilities where needed as well as dropping the old SMIA
limits.

Remove macros for defining new SMIA registers, instead put the register
flags to the register definition itself. Also move the register flags to
the same file.
This is not expected to be updated but rather left there as a reference.

include/media/i2c/smiapp.h was meant to serve systems where the sensor is
enumerated through platform data. That's no longer necessary, hopefully
not even in out-of-tree use cases. Move the definitions to the appropriate
headers.

Instead of keeping track of the power state ourselves, let runtime PM
handle it.
This also splits handling controls between side effect management and
writing the new configuration to the sensor's registers.

The interface supports probing for the NVM size but this was not
implemented in the driver. Do that now.
This will also make nokia,nvm-size property redundant.

Instead of relying on a mix of runtime PM and the s_power() callback, drop
the s_power() callback altogether and use runtime PM solely.
As device access is required during device power-on and power-off
sequences, runtime PM alone cannot tell whether the device is available.
Thus the "active" field is introduced in struct smiapp_sensor to tell
whether it is safe to write to the device.
Consequently there is no need to power on the device whenever a file
handle is open. This functionality is removed as well. The user may still
control the device power management through sysfs. Autosuspend remains
enabled, with 1 s delay.

Besides the image data, SMIA++ compliant sensors also provide embedded
data in form of registers used to capture the image. Store this
information for later use in frame descriptor and routing.

Some sensors (like the one in Nokia N900) provide metadata in the first
couple of lines. Make that information information available to the
pipeline.
Use u16 instead, this is a 16-bit value.

Currently, include/media is messy, as it contains both the V4L2 core
headers and some driver-specific headers on the same place. That makes
harder to identify what core headers should be documented and what
headers belong to I2C drivers that are included only by bridge/main
drivers that would require the functions provided by them.
Let's move those i2c specific files to its own subdirectory.
The files to move were produced via the following script:

Remove FSF's address information from the license header in the smiapp
driver and the smiapp-pll PLL calculator. This should no longer be needed,
and would be rendered outdated in case the FSF chooses to relocate its
office.

Not all link rates are possible with all BPP values.
Also rearrange other initialisation a little. Obtaining possible PLL
configurations earlier requires that.
