Pull locking and atomic updates from Ingo Molnar:
"Main changes in this cycle are:
- Extend atomic primitives with coherent logic op primitives
(atomic_{or,and,xor}()) and deprecate the old partial APIs
(atomic_{set,clear}_mask())
The old ops were incoherent with incompatible signatures across
architectures and with incomplete support. Now every architecture
supports the primitives consistently (by Peter Zijlstra)
- Generic support for 'relaxed atomics':
- _acquire/release/relaxed() flavours of xchg(), cmpxchg() and {add,sub}_return()
- atomic_read_acquire()
- atomic_set_release()
This came out of porting qwrlock code to arm64 (by Will Deacon)
- Clean up the fragile static_key APIs that were causing repeat bugs,
by introducing a new one:
DEFINE_STATIC_KEY_TRUE(name);
DEFINE_STATIC_KEY_FALSE(name);
which define a key of different types with an initial true/false
value.
Then allow:
static_branch_likely()
static_branch_unlikely()
to take a key of either type and emit the right instruction for the
case. To be able to know the 'type' of the static key we encode it
in the jump entry (by Peter Zijlstra)
- Static key self-tests (by Jason Baron)
- qrwlock optimizations (by Waiman Long)
- small futex enhancements (by Davidlohr Bueso)
- ... and misc other changes"

Allocating a large number of elements in atomic context could quickly
deplete memory reserves, so just disallow atomic resizing entirely.
Nothing currently uses mempool_resize() with anything other than
GFP_KERNEL, so convert existing callers to drop the gfp_mask.

All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.
Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

This patch improves the Fibre Channel port scan behaviour of the zfcp lldd.
Without it the zfcp device driver may churn up the storage area network by
excessive scanning and scan bursts, particularly in big virtual server
environments, potentially resulting in interference of virtual servers and
reduced availability of storage connectivity.
The two main issues as to the zfcp device drivers automatic port scan in
virtual server environments are frequency and simultaneity.
On the one hand, there is no point in allowing lots of ports scans
in a row. It makes sense, though, to make sure that a scan is conducted
eventually if there has been any indication for potential SAN changes.
On the other hand, lots of virtual servers receiving the same indication
for a SAN change had better not attempt to conduct a scan instantly,
that is, at the same time.
Hence this patch has a two-fold approach for better port scanning:
the introduction of a rate limit to amend frequency issues, and the
introduction of a short random backoff to amend simultaneity issues.
Both approaches boil down to deferred port scans, with delays
comprising parts for both approaches.
The new port scan behaviour is summarised best by:

Implementation is straight-forward by converting an existing worker to
a delayed worker. But care is needed whenever that worker is going to be
flushed (in order to make sure work has been completed), since a flush
operation cancels the timer set up for deferred execution (see * above).
There is a small race window whenever a port scan work starts
running up to the point in time of storing the time stamp for that port
scan. The impact is negligible. Closing that gap isn't trivial, though, and
would the destroy the beauty of a simple work-to-delayed-work conversion.

Through sysfs attributes, zfcp unit objects
provide a trigger for manual LUN recovery
and export information for problem determination.
With commit
f8210e34887e1feb977a9b6b8caa086855af40c9
"[SCSI] zfcp: Allow midlayer to scan for LUNs when running in NPIV mode"
and when attaching SCSI devices through this new optional method,
no more zfcp unit objects are allocated for such SCSI devices.
Hence, the above-mentioned trigger and information were missing.
The information and context is located in SCSI transport device data since

Hence, introduce the trigger and the information unconditionally
for all SCSI devices attached through zfcp.
We prefix the attribute names with 'zfcp_' to prevent collisions and
to avoid mix-ups such as with the common 'state' attribute.
Since some of the new attribute views do not need zfcp_port
in the ZFCP_DEFINE_SCSI_ATTR helper macro, remove zfcp_port
to avoid compiler warnings on unused variable.
It's easy to open code the conversion from zfcp_scsi_dev to zfcp_port
for the two already existing attributes hba_id and wwpn.

This patch removes some leftovers for commit

The "access denied" case for ports is gone, as well.
The corresponding flag was cleared, but never set.
So clean it up.
Sysfs flag is kept, though, for backward-compatibility.
Now it returns always 0.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.
Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

employing large or hierarchical LUN numbers become more and more
common.

Enabling the data router support by default
can increase performance in certain situations.
It is safe to do so and tolerated in LPAR and under z/VM
in case there is no data router support in that environment.

By popular demand, this patch brings back a couple of sysfs attributes
removed by commit 663e0890e31cb85f0cca5ac1faaee0d2d52880b5
"[SCSI] zfcp: remove access control tables interface".
The content has been irrelevant for years, but the files must be
there forever for whatever user space tools that may rely on them.
Since these files always return a constant value, a new stripped
down show-macro was required. Otherwise build warnings would have
been introduced.

Apparently, the ref_count for some scsi_device drops down to zero,
triggering device removal through execute_in_process_context(), while
the lldd error recovery thread iterates through a scsi device list.
Unfortunately, execute_in_process_context() decides to immediately
execute that device removal function, instead of scheduling asynchronous
execution, since it detects process context and thinks it is safe to do
so. But almost all calls to shost_for_each_device() in our lldd are
inside spin_lock_irq, even in thread context. Obviously, schedule()
inside spin_lock_irq sections is a bad idea.
Change the lldd to use the proper iterator function,
__shost_for_each_device(), in combination with required locking.
Occurences that need to be changed include all calls in zfcp_erp.c,
since those might be executed in zfcp error recovery thread context
with a lock held.
Other occurences of shost_for_each_device() in zfcp_fsf.c do not
need to be changed (no process context, no surrounding locking).
The problem was introduced in Linux 2.6.37 by commit

This patch adds wait_event_interruptible_lock_irq_timeout(), which is a
straight-forward descendant of wait_event_interruptible_timeout() and
wait_event_interruptible_lock_irq().
The zfcp driver used to call wait_event_interruptible_timeout()
in combination with some intricate and error-prone locking. Using
wait_event_interruptible_lock_irq_timeout() as a replacement
nicely cleans up that locking.
This rework removes a situation that resulted in a locking imbalance
in zfcp_qdio_sbal_get():

It was introduced by commit c2af7545aaff3495d9bf9a7608c52f0af86fb194
"[SCSI] zfcp: Do not wait for SBALs on stopped queue", which had a new
code path related to ZFCP_STATUS_ADAPTER_QDIOUP that took an early exit
without a required lock being held. The problem occured when a
special, non-SCSI I/O request was being submitted in process context,
when the adapter's queues had been torn down. In this case the bug
surfaced when the Fibre Channel port connection for a well-known address
was closed during a concurrent adapter shut-down procedure, which is a
rare constellation.
This patch also fixes these warnings from the sparse tool (make C=1):

started using a value returned by the channel but only evaluated the value
if the fabric link is up.

introduced mempool resizings based on the above value.
On setting an FCP device online for the very first time since boot, a new
zeroed adapter object is allocated. If the link is down, the number of
status read requests remains zero. Since just the config data exchange is
incomplete, we proceed with adapter open recovery. However, we
unconditionally call mempool_resize with adapter->stat_read_buf_num == 0 in
this case.
This causes a kernel message "kernel BUG at mm/mempool.c:131!" in process
"zfcperp<FCP-device-bus-ID>" with last function mempool_resize in Krnl PSW
and zfcp_erp_thread in the Call Trace.
Don't evaluate channel values which are invalid on link down. The number of
status read requests is always valid, evaluated, and set to a positive
minimum greater than zero. The adapter open recovery can proceed and the
channel has status read buffers to inform us on a future link up event.
While we are not aware of any other code path that could result in mempool
resize attempts of size zero, we still also initialize the number of status
read buffers to be posted to a static minimum number on adapter object
allocation.

This patch removes an interface that was used to manage access control
tables within the HBA. The patch consequently removes the handling
for conditions related to those access control tables, too.
That initiator-based access control feature was only needed until the
introduction of NPIV and was withdrawn with z10 years ago.
It's time to cleanup the corresponding device driver code.

So far, we could only increase the s390dbf log level after an FCP
device has been initially set online for it to create the dbf entries
required to adjust the level.
Introduce zfcp.dbflevel as counterpart to the already existing
zfcp.dbfsize to enable debugging of e.g. setting an FCP device online.

"[SCSI] zfcp: support for hardware data router"
reduced the initial block queue limits in the scsi_host_template to the
absolute minimum and adjusted them later on. However, the adjustment was
too late for the BSG devices of Scsi_Host and fc_host.
Therefore, ioctl(..., SG_IO, ...) with request or response size > 4kB to a
BSG device of an fc_host or a Scsi_Host fails with EINVAL. As a result,
users of such ioctl such as HBA_SendCTPassThru() in libzfcphbaapi return
with error HBA_STATUS_ERROR.
Initialize the block queue limits in zfcp_scsi_host_template to the
greatest common denominator (GCD).
While we cannot exploit the slightly enlarged maximum request size with
data router, this should be neglectible. Doing so also avoids running into
trouble after live guest relocation (LGR) / migration from a data router
FCP device to an FCP device that does not support data router. In that
case, zfcp would figure out the new limits on adapter recovery, but the
fc_host and Scsi_Host (plus in fact all sdevs) still exist with the old and
now too large queue limits.
It should also OK, not to use half the size as in the DIX case, because
fc_host and Scsi_Host do not transport FCP requests including SCSI commands
using protection data.

FCP device remains in status ERP_FAILED when device is switched online
or adapter recovery is triggered while link to SAN is down.
When Exchange Configuration Data command returns the FSF status
FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE it aborts the exchange process.
The only retries are done during the common error recovery procedure
(i.e. max. 3 retries with 8sec sleep between) and remains in status
ERP_FAILED with QDIO down.
This commit reverts the commit 0df138476c8306478d6e726f044868b4bccf411c
(zfcp: Fix adapter activation on link down).
When FSF status FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE is received the
adapter recovery will be finished without any retries. QDIO will be
up now and status changes such as LINK UP will be received now.

__scsi_remove_device (e.g. due to dev_loss_tmo) calls
zfcp_scsi_slave_destroy which in turn sends a close LUN FSF request to
the adapter. After 30 seconds without response,
zfcp_erp_timeout_handler kicks the ERP thread failing the close LUN
ERP action. zfcp_erp_wait in zfcp_erp_lun_shutdown_wait and thus
zfcp_scsi_slave_destroy returns and then scsi_device is no longer
valid. Sometime later the response to the close LUN FSF request may
finally come in. However, commit
b62a8d9b45b971a67a0f8413338c230e3117dff5
"[SCSI] zfcp: Use SCSI device data zfcp_scsi_dev instead of zfcp_unit"
introduced a number of attempts to unconditionally access struct
zfcp_scsi_dev through struct scsi_device causing a use-after-free.
This leads to an Oops due to kernel page fault in one of:
zfcp_fsf_abort_fcp_command_handler, zfcp_fsf_open_lun_handler,
zfcp_fsf_close_lun_handler, zfcp_fsf_req_trace,
zfcp_fsf_fcp_handler_common.
Move dereferencing of zfcp private data zfcp_scsi_dev allocated in
scsi_device via scsi_transport_reserve_device after the check for
potentially aborted FSF request and thus no longer valid scsi_device.
Only then assign sdev_to_zfcp(sdev) to the local auto variable struct
zfcp_scsi_dev *zfcp_sdev.

In FC fabrics with large zones, the automatic port_rescan on incoming ELS
and any adapter recovery can cause quite some traffic at the very same
time, especially if lots of Linux images share an HBA, which is common on
s390. This can cause trouble and failures. Fix this by making such port
rescans dependent on a user configurable module parameter.
The following unconditional automatic port rescans remain as is:
On setting an adapter online and
on manual user-triggered writes to the sysfs attribute port_rescan.

accidentally dropped a reference count check before tearing down
zfcp_ports that are potentially in use by zfcp_units.
Even remote ports in use can be removed causing
unreachable garbage objects zfcp_ports with zfcp_units.
Thus units won't come back even after a manual port_rescan.
The kref of zfcp_port->dev.kobj is already used by the driver core.
We cannot re-use it to track the number of zfcp_units.
Re-introduce our own counter for units per port
and check on port_remove.

If list_for_each_entry, etc complete a traversal of the list, the iterator
variable ends up pointing to an address at an offset from the list head,
and not a meaningful structure. Thus this value should not be used after
the end of the iterator. Replace port->adapter->scsi_host by
adapter->scsi_host.
This problem was found using Coccinelle (http://coccinelle.lip6.fr/).

If the mapping of FCP device bus ID and corresponding subchannel
is modified while the Linux image is suspended, the resume of FCP
devices can fail. During resume, zfcp gets callbacks from cio regarding
the modified subchannels but they can be arbitrarily mixed with the
restore/resume callback. Since the cio callbacks would trigger
adapter recovery, zfcp could wakeup before the resume callback.
Therefore, ignore the cio callbacks regarding subchannels while
being suspended. We can safely do so, since zfcp does not deal itself
with subchannels. For problem determination purposes, we still trace the
ignored callback events.
The following kernel messages could be seen on resume:
kernel: <WWPN>: parent <FCP device bus ID> should not be sleeping
As part of adapter reopen recovery, zfcp performs auto port scanning
which can erroneously try to register new remote ports with
scsi_transport_fc and the device core code complains about the parent
(adapter) still sleeping.

ERP cannot recover an error on the FCP device
In such cases, the adapter gave up recovery and remained blocked along
with its child objects: remote ports and LUNs/scsi devices. Even the
adapter shutdown as part of giving up recovery failed because the ccw
device state remained disconnected. Later, the corresponding remote
ports ran into dev_loss_tmo. As a result, the LUNs were erroneously
not available again after resume.
Even a manually triggered adapter recovery (e.g. sysfs attribute
failed, or device offline/online via sysfs) could not recover the
adapter due to the remaining disconnected state of the corresponding
ccw device.

The pl vector has scount elements, i.e. pl[scount-1] is the last valid
element. For maximum sized requests, payload->counter == scount after
the last loop iteration. Therefore, do bounds checking first (with
boolean shortcut) to not access the invalid element pl[scount].
Do not trust the maximum sbale->scount value from the HBA
but ensure we won't access the pl vector out of our allocated bounds.
While at it, clean up scoping and prevent unnecessary memset.

changed the semantics of FC_PORTSPEED defines to
FDMI port attributes of FC-HBA/SM-HBA
which is different from the previous bit reversed
Report Port Speed Capabilities (RPSC) ELS of FC-GS/FC-LS.
Zfcp showed "10 Gbit" instead of "4 Gbit" for supported_speeds.
It now uses explicit bit conversion as the other LLDs already
do, in order to be independent of the kernel bit semantics.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

The new is_compat_task() define for the !COMPAT case in
include/linux/compat.h conflicts with a similar define in
arch/s390/include/asm/compat.h.
This is the minimal patch which fixes the build issues.

zfcp_scsi_slave_destroy erroneously always tried to finish its task
even if the corresponding previous zfcp_scsi_slave_alloc returned
early. This can lead to kernel page faults on accessing uninitialized
fields of struct zfcp_scsi_dev in zfcp_erp_lun_shutdown_wait. Take the
port field of the struct to determine if slave_alloc returned early.
This zfcp bug is exposed by 4e6c82b (in turn fixing f7c9c6b to be
compatible with 21208ae) which can call slave_destroy for a
corresponding previous slave_alloc that did not finish.
This patch is based on James Bottomley's fix suggestion in
http://www.spinics.net/lists/linux-scsi/msg55449.html.

Fix several compile errors on s390 caused by splitting module.h.
