The mlx4 WQ is implemented with HW QP without special HW object. Current
implementation which tried to reuse the code did it with common QP
creation flows. Such decision caused to the absence of mlx4_ib_wq struct,
which is needed to ensure proper allocation of ib_wq inside of IB/core.
Separate create_qp_common() to pure QP flow and to create_rq() for RWQ.

Instead of RoCE drivers figuring out vlan, smac fields while working on
QP/AH, provide a helper routine to read the L2 fields such as vlan_id and
source mac address.
This moves logic from mlx5 driver to core for wider usage for RoCE ports.
This is a preparation patch to allow detaching netdev in subsequent patch.

mmiowb() is now implied by spin_unlock() on architectures that require
it, so there is no reason to call it from driver code. This patch was

NOTE: mmiowb() has only ever guaranteed ordering in conjunction with
spin_unlock(). However, pairing each mmiowb() removal in this patch with
the corresponding call to spin_unlock() is not at all trivial, so there
is a small chance that this change may regress any drivers incorrectly
relying on mmiowb() to order MMIO writes between CPUs using lock-free
synchronisation. If you've ended up bisecting to this commit, you can
reintroduce the mmiowb() calls using wmb() instead, which should restore
the old behaviour on all architectures other than some esoteric ia64
systems.

Now when ib_udata is passed to all the driver's object create/destroy APIs
the ib_udata will carry the ib_ucontext for every user command. There is
no need to also pass the ib_ucontext via the functions prototypes.
Make ib_udata the only argument psssed.

The uverbs_attr_bundle with the ucontext is sent down to the drivers ib_x
destroy path as ib_udata. The next patch will use the ib_udata to free the
drivers destroy path from the dependency in 'uobject->context' as we
already did for the create path.

ib_umem_get() can only be called in a method callback, which always has a
udata parameter. This allows ib_umem_get() to derive the ucontext pointer
directly from the udata without requiring the drivers to find it in some
way or another.

Drivers should be using udata to determine if a method is invoked from
user space or kernel space. A pd does not necessarily say a different
objects is kernel or user.
Transforming the tests to use udata eliminates a large number of uobject
references from the drivers.

In the current implementation, the driver tries to allocate contiguous
memory, and if it fails, it falls back to 4K fragmented allocation.
Once the memory is fragmented, the first allocation might take a lot
of time, and even fail, which can cause connection failures.
This patch changes the logic to always allocate with 4K granularity,
since it's more robust and more likely to succeed.
This patch was tested with Lustre and no performance degradation
was observed.
Note: This commit eliminates the "shrinking WQE" feature. This feature
depended on using vmap to create a virtually contiguous send WQ.
vmap use was abandoned due to problems with several processors (see the
commit cited below). As a result, shrinking WQE was available only with
physically contiguous send WQs. Allocating such send WQs caused the
problems described above.
Therefore, as a side effect of eliminating the use of large physically
contiguous send WQs, the shrinking WQE feature became unavailable.

Since neither ib_post_send() nor ib_post_recv() modify the data structure
their second argument points at, declare that argument const. This change
makes it necessary to declare the 'bad_wr' argument const too and also to
modify all ULPs that call ib_post_send(), ib_post_recv() or
ib_post_srq_recv(). This patch does not change any functionality but makes
it possible for the compiler to verify whether the
ib_post_(send|recv|srq_recv) really do not modify the posted work request.
To make this possible, only one cast had to be introduce that casts away
constness, namely in rpcrdma_post_recvs(). The only way I can think of to
avoid that cast is to introduce an additional loop in that function or to
change the data type of bad_wr from struct ib_recv_wr ** into int
(an index that refers to an element in the work request list). However,
both approaches would require even more extensive changes than this
patch.

When posting a send work request, the work request that is posted is not
modified by any of the RDMA drivers. Make this explicit by constifying
most ib_send_wr pointers in RDMA transport drivers.

This patch follows the logic from ib_core but considers the internal
device state upon executing the involved commands.
Specifically, Upon internal error state modify QP to an error state can
be assumed to be success as each in-progress WR going to be flushed in
error in any case as expected by that modify command.
In addition,
As the drain should never fail the driver makes sure that post_send/recv
will succeed even if the device is already in an internal error state.
As such once the driver will supply the simulated/SW CQEs the CQE for
the drain WR will be handled as well.
In case of an internal error state the CQE for the drain WR may be
completed as part of the main task that handled the error state or by
the task that issued the drain WR.
As the above depends on scheduling the code takes the relevant locks
and actions to make sure that the completion handler for that WR will
always be called after that the post_send/recv were issued but not in
parallel to the other task that handles the error flow.

For UD the drivers were doing a sgid_index lookup into the cache to get
the attrs, however we can now directly access the same attrs stores in
the ib_ah instead and remove the lookup.

The core code now ensures that all driver callbacks that receive an
rdma_ah_attrs will have a sgid_attr's pointer if there is a GRH present.
Drivers can use this pointer instead of calling a query function with
sgid_index. This simplifies the drivers and also avoids races where a
gid_index lookup may return different data if it is changed.

The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:
kmalloc(a * b, gfp)
with:
kmalloc_array(a * b, gfp)
as well as handling cases of:
kmalloc(a * b * c, gfp)
with:
kmalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kmalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kmalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.
The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

Code includes wmb() followed by writel(). writel() already has a barrier on
some architectures like arm64.
This ends up CPU observing two barriers back to back before executing the
register write.
Since code already has an explicit barrier call, changing writel() to
writel_relaxed().

As a default, for Ethernet packets, the device scatters only the payload
of ingress packets. The scatter FCS feature lets the user to get the FCS
(Ethernet's frame check sequence) in the received WR's buffer as a 4
Bytes trailer following the packet's payload.

Support RSS hash for inner headers according to a new flag,
MLX4_IB_RX_HASH_INNER provided by the vendor channel.
In case the flag is set, RSS hash will be done on the inner headers of
VXLAN packets (which are encapsulated).
Non-encapsulated packets will be hashed according to the outer headers.

The mlx4_ib_gid_index_to_real_index() does check that "port" is within

Mistakenly the driver didn't allow RSS hash fields combinations which
involve both IPv4 and IPv6 protocols. This bug caused to failures for
user's use cases for RSS.
Consequently, this patch fixes this bug and allows any combination that
the HW can support.
Additionally, the patch fixes the driver to return an error in case the
user provides an unsupported mask for RSS hash fields.

Maximal message should be used as a limit to the max message payload allowed,
without the headers. The ConnectX-3 check is done against this value includes
the headers. When the payload is 4K this will cause the NIC to drop packets.
Increase maximal message to 8K as workaround, this shouldn't change current
behaviour because we continue to set the MTU to 4k.
To reproduce;
set MTU to 4296 on the corresponding interface, for example:
ifconfig eth0 mtu 4296 (both server and client)
On server:
ib_send_bw -c UD -d mlx4_0 -s 4096 -n 1000000 -i1 -m 4096
On client:
ib_send_bw -d mlx4_0 -c UD <server_ip> -s 4096 -n 1000000 -i 1 -m 4096

In the modify QP handler the base_qpn_udp field in the RSS QPC is
overwrite later by irrelevant value assignment. Hence, ingress packets
which gets to the RSS QP will be steered then to a garbage QPN.
The patch fixes this by skipping the above assignment when a RSS QP is
modified, also, the RSS context's attributes assignments are relocated
just before the context is posted to avoid future issues like this.
Additionally, this patch takes the opportunity to change the code to be
disciplined to the device's manual and assigns the RSS QP context just at
RESET to INIT transition.

Add support to work with a RSS QP by using an indirection table object
upon QP creation. Other related QP verbs (e.g. modify/destroy/query) were
updated as well for that QP mode.
Notes:
- The RX hash properties are supplied as driver private data.
- The RSS QP port is used on the associated WQs in its indirection
table. Applying different ports during WQ life time is not allowed.
- The expected RSS QP flow is: create, modify(RST->INIT),
modify(RST->RTR), destroy.

To enable RSS functionality the IB indirection table object (i.e.
ib_rwq_ind_table) should be used.
This patch implements the related verbs as of create and destroy an
indirection table.
In downstream patches the indirection table will be used as part of RSS
QP creation.
