The ti_clk_register() and ti_clk_register_omap_hw() functions are always
called with the parameter of type "struct device" set to NULL, since the
functions from which they are called always have a parameter of type
"struct device_node". Replacing "struct device" type parameter with
"struct device_node" will allow you to register a TI clock to the common
clock framework by taking advantage of the facilities provided by the
"struct device_node" type. Further, adding the "of_" prefix to the name
of these functions explicitly binds them to the "struct device_node"
type.
The patch has been tested on a Beaglebone board.

The patch enables spread spectrum clocking (SSC) for MPU and LCD PLLs.
As reported by the TI spruh73x/spruhl7x RM, SSC is only supported for
the DISP/LCD and MPU PLLs on am33xx/am43xx. SSC is not supported for
DDR, PER, and CORE PLLs.
Calculating the required values and setting the registers accordingly
was taken from the set_mpu_spreadspectrum routine contained in the
arch/arm/mach-omap2/am33xx/clock_am33xx.c file of the u-boot project.
In locked condition, DPLL output clock = CLKINP *[M/N]. In case of
SSC enabled, the reference manual explains that there is a restriction
of range of M values. Since the omap2_dpll_round_rate routine attempts
to select the minimum possible N, the value of M obtained is not
guaranteed to be within the range required. With the new "ti,min-div"
parameter it is possible to increase N and consequently M to satisfy the
constraint imposed by SSC.

kmemdup is introduced to duplicate a region of memory in a neat way.
Rather than kmalloc/kzalloc + memcpy, which the programmer needs to
write the size twice (sometimes lead to mistakes), kmemdup improves
readability, leads to smaller code and also reduce the chances of mistakes.
Suggestion to use kmemdup rather than using kmalloc/kzalloc + memcpy.

A future patch is going to change semantics of clk_register() so that
clk_hw::init is guaranteed to be NULL after a clk is registered. Avoid
referencing this member here so that we don't run into NULL pointer
exceptions.

Add a generic API for initializing clocks of clk_hw_omap type clocks,
and convert the whole TI clock driver suite to use this for registering
the clocks. Also, get rid of the now redundant API for adding the clocks
to the OMAP HW clocks list; instead this is used directly from the
register API.

Convert string compares of DT node names to use of_node_name_eq helper
instead. This removes direct access to the node name pointer.
For instances using of_node_cmp, this has the side effect of now using
case sensitive comparisons. This should not matter for any FDT based
system which all of these are.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:
kzalloc(a * b, gfp)
with:
kcalloc(a * b, gfp)
as well as handling cases of:
kzalloc(a * b * c, gfp)
with:
kzalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kzalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kzalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

When none of the OMAP4-generation SoCs are enabled, we run into a link
error for am43xx/am43xx:
drivers/clk/ti/dpll.o: In function `of_ti_am3_dpll_x2_setup':
dpll.c:(.init.text+0xd8): undefined reference to `clkhwops_omap4_dpllmx'
This is easily fixed by adding another #ifdef.
While looking at the code, I also spotted another problem with the
assignment of hw_ops variable that is not used again later. I'm
changing this to setting clk_hw->ops instead, which I guess is what
was intended here.

Currently, TI clock driver uses an encapsulated struct that is cast into
a void pointer to store all register addresses. This can be considered
as rather nasty hackery, and prevents from expanding the register
address field also. Instead, replace all the code to use proper struct
in place for this, which contains all the previously used data.
This patch is rather large as it is touching multiple files, but this
can't be split up as we need to avoid any boot breakage.

The OMAP36xx DPLL5, driving EHCI USB, can be subject to a long-term
frequency drift. The frequency drift magnitude depends on the VCO update
rate, which is inversely proportional to the PLL divider. The kernel
DPLL configuration code results in a high value for the divider, leading
to a long term drift high enough to cause USB transmission errors. In
the worst case the USB PHY's ULPI interface can stop responding,
breaking USB operation completely. This manifests itself on the
Beagleboard xM by the LAN9514 reporting 'Cannot enable port 2. Maybe the
cable is bad?' in the kernel log.
Errata sprz319 advisory 2.1 documents PLL values that minimize the
drift. Use them automatically when DPLL5 is used for USB operation,
which we detect based on the requested clock rate. The clock framework
will still compute the PLL parameters and resulting rate as usual, but
the PLL M and N values will then be overridden. This can result in the
effective clock rate being slightly different than the rate cached by
the clock framework, but won't cause any adverse effect to USB
operation.

AM33xx/AM43xx devices use the same DPLL IP blocks, which only support
maximum rate of 1GHz [1] for the default and 2GHz for the low-jitter type
DPLLs [2]. Reflect this limitation in the DPLL init code by adding the
max-rate parameter based on the DPLL types.

Convert DPLL support code to use clk_hw pointers for reference and bypass
clocks. This allows us to use clk_hw_* APIs for accessing any required
parameters for these clocks, avoiding some locking problems at least with
DPLL enable code; this used clk_get_rate which uses mutex but isn't
good under clk_enable / clk_disable.

There is a case where NULL can be a valid return value for
ti_clk_get_reg_addr, specifically the case where both the provider index
and register offsets are zero. In this case, the current error checking
against a NULL pointer will fail. Thus, change the API to return a
ERR_PTR value in an error case, and change all the users of this API to
check against IS_ERR instead.

Currently, DPLLs are hiding the gory details of switching parent
within set_rate, which confuses the common clock code and is wrong.
Fixed by applying the new determine_rate() and set_rate_and_parent()
functionality to any clock-ops previously using the broken approach.
This patch also removes the broken legacy code.

MPU DPLL on OMAP5, DRA75x, DRA72x has a limitation on the maximum
frequency it can be locked at. Duty Cycle Correction circuit is used
to recover a correct duty cycle for achieving higher frequencies
(hardware internally switches output to M3 output(CLKOUTHIF) from M2
output (CLKOUT)).
So provide support to setup required data to handle Duty cycle by
the setting up the minimum frequency for DPLL. 1.4GHz is common
for all these devices and is based on Technical Reference Manual
information for OMAP5432((SWPU282U) chapter 3.6.3.3.1 "DPLLs Output
Clocks Parameters", and equivalent information from DRA75x, DRA72x
documentation(SPRUHP2E, SPRUHI2P).

AMxxxx dpll_data previously had autoidle_mask set, even if these SoC:s
don't have autoidle register. Remove the bit-field value as it is unused,
also drop the unnecessary DPLL_HAS_AUTOIDLE flag passing during init,
as we can just simply check against the contents of the autoidle_mask.
